Ride Connection is a mobility management program to provide non-emergency medical transportation and transportation counseling to eligible participants in the service area.
The transportation service has expanded. Transportation can be provided for up to four trips each calendar month for medical and dental appointments, pharmacy visits, and shopping for food (grocery store or food pantry). If you have access to Medicaid medical transportation, you are not eligible to receive rides to medical appointments through Ride Connection, but they can assist with shopping for food.

Must reside and travel within the Planning District 15 service area.
Other Eligibility Criteria
Participants must be at least 60 years old or under 60 years old receiving disability benefits from the Social Security Administration.

Intake Process
Accepts referrals from Senior Connections Care Coordinators, board members, and others. Clients can be registered over the phone and sent a written copy of the Guidelines and Policies.
