Diabetes Self-Management Program (DSMP) is designed for individuals that have been diagnosed with diabetes or pre diabetes. The program will teach individuals how to manage their blood sugar through diet, exercise and possibly medication.
Participants will have information and encouragement that are needed to prevent pre diabetes from progressing to diabetes, or to manage their diabetes.
