Who We Are
Centra PACE provides a program of health and social services to seniors who qualify for nursing facility level of care but want to stay in their homes. An interdisciplinary team of professionals made up of primary care physicians, registered nurses, social workers, health aides, transportation personnel, rehabilitation therapists, dietitians, and recreation/activity personnel work with a participant and his or her family to develop a tailored plan of care which is monitored closely and updated as needed.
PACE participants can be picked up from their homes each morning and brought to the PACE Center. The Center provides socialization, hot lunch, and medical assistance as needed. A PACE participant must live in the city of Lynchburg or surrounding communities within 30 minutes of downtown Lynchburg.

Medical care and transportation to and from specialists
Adult daycare
Hospital care. (PACE team will coordinate care)
Prescription medicines
Vision, hearing, and dental care
Rehabilitation and therapy services
Durable medical equipment
Outpatient medical services: lab work, X-rays, etc.
Mental health care
Home care and nutritional services
Respite services
24-hour on-call physician services
Meals
Personal emergency response
Wheelchair accessible transportation
A participant of Centra PACE receives all health care services arranged by the interdisciplinary team. If a participant receives medical services from an out-of-network provider or without prior authorization, he or she may be liable for the full cost of those services. Capacity is 75.
