Disease Prevention and Health Promotion

The Disease Prevention and Health Promotion Program provides activities designed to prevent disease and promote the health of older persons. These include health risk assessments; routine health screening; nutritional counseling and education; health promotion; physical fitness and group exercise; music/are/dance-movement therapy; home injury control; mental health screening/education/referral; information on Medicare coverage; medication management; information on diagnosis/prevention/treatment/rehabilitation of age related diseases and chronic disabling conditions; gerontological counseling; and social services/follow up health services counseling. These programs are offered as all nutrition sites, seniors and senior clubs upon request. Services are available to home bound clients.
